Eley Photo will award a preloaded Visa gift card to any Eligible Referrer who refers an Eligible New Customer in compliance with these Referral Program Rules (the “Reward”). The Reward will be calculated based on the number of lines and Eley Photo Product(s) selected by the Eligible New Customer for activation or installation. The one-time Reward will be issued after all of the following have occurred: (a) the Eligible New Customer's account has been active and in good standing for at least ninety (90) days; (b) Eley Photo receives full payment from the Eligible New Customer for the three (3) consecutive months for the requisite extensions; and (c) the Eligible Referrer has submitted a complete and accurate W-9.
